Disclaimer: Hazel Health is currently (23-24 school year) being piloted at Parks, Rolling Hills and MyConnect Academy for mental health referrals/appointments during school/after school hours. Tele-therapy at school will be available to all sites during the 24-25 school year.

All families can request Hazel Health tele-therapy services for at home, after school hours, Monday - Friday until 7pm. All families will have access during summer months as well.  For at home tele-therapy services, after school hours, please fill out the consent form on the right.
