    Rates are changing. Please make sure to check rate sheets for costs effective October 1, 2025. Rates can be found here
     
    25/26 Open Enrollment Plan Changes:
    1. Blue Shield HMO's (10, 30, and TRIO) - No plan changes, coverage remains the same.
    Rates have changed, please see rate sheet for more information.
     
    Coverage IS changing;
        • Deductible: $500 individual / $1,000 family (change) (24/25 PPO plan deductible: $300 individual / $600 family)
        • Calendar Year Out-of-Pocket max: $2,000 individual / $4,000 family (change) (24/25 PPO plan Out of Pocket Max: $1,000 individual / $3,000 family)
        • Coinsurance: 20% after deductible (remains the same)
    3. Blue Shield PPO HSA - No plan changes, coverage remains the same.

    4. Kaiser 15 and Kaiser 30 - No plan changes, coverage remains the same.
     Rates have changed, please see rate sheet for more information.
     
    5. Proactive Care Plan, PPO - New plan!!

    • $0 Primary Care Visits (includes Centivo)
    • $0 Urgent Care Visits (includes MDLive)
    • $0 Physical Therapy
    • $0 Lab Work
    • $0 Mental Health
    • $0 some Brand Medications when prescribed by a PCP
      • Asthma inhalers like Qvar, Arnuity Ellipta, Albuterol
      • Diabetic medications like Ozempic, Jardiance, Mounjaro, Rybelsus, Trulicity
      • Insulins like insulin lispro, Humalog, Semglee
      • Anticoagulants like Xarelto, Eliquis
      • Specialty Medicines like Dupixent, Xolair
      • Biosimilars like Adalimumab-adaz, Hadlima
     
  • All plan and coverage changes will be effective October 1, 2025.

    If you do not want to make any changes you do NOT need to turn in anything. 
     
    If nothing is received during Open Enrollment, enrolled benefit plans will remain the same (with the exception of Medical and Dependent FSA). Only applicable rate increases and/or plan changes will apply.
