California Healthy Kids Survey Administration
During the week of December 4 - December 8, 2023, the California Healthy Kids Survey (CHKS) will be administered to all Fullerton School District students in the 5th and 7th grades. Students in 7th grade will only be administered Module A of the survey. The CHKS is sponsored by the California Department of Education and meets a requirement of the Title IV Safe and Drug-Free Schools and Communities.
The CHKS allows districts to identify the health and safety needs of students, establish program goals, and monitor progress in achieving goals. It is designed to be part of a comprehensive decision-making process to help guide the development of more effective health, prevention and youth development programs, such as Safe Schools and the mandated wellness policy.
Parent consent and withdrawal forms and additional information will be sent home with all 5th and 7th grade students. This survey is required by the State, however, parents may opt-out for their child by submitting the proper form. Parents of 5th-grade students must return the consent form if they choose to have their students participate in the survey. The survey will be administered to all 7th grade students unless their parent(s) contact the school office and request to receive and complete the withdrawal form.
